Care Manager RN - Foster Care

Job Description

We are looking for a compassionate and skilled Care Manager RN specializing in Foster Care to join our team at Superior HealthPlan. As a member of our Medical Management/Health Services team, you will play a vital role in assessing, planning, and coordinating medical and support services to ensure quality and cost-effective care for select members.

Responsibilities

• Assess members' health status, treatment plans, and goals to develop personalized care plans.
• Coordinate services with healthcare providers to meet members' medical and socio-economic needs.
• Provide patient and provider education, and facilitate access to community-based services.
• Participate in care management rounds and identify risk management concerns.
• Maintain clinical information in medical management systems and ensure compliance with care plan requirements.

Requirements

• Graduate from an Accredited School of Nursing with a current RN license.
• 2+ years of clinical nursing experience and 1+ years of case management experience in a managed care setting.
• Knowledge of utilization management principles and healthcare managed care.
• Experience with medical decision support tools and government-sponsored managed care programs.

Salary

Pay Range: $66,100.00 - $118,900.00 per year
