HOME CARE PHYSICAL THERAPIST (PT)

Job Description

Liberty Healthcare Management is currently seeking a dedicated Home Care Physical Therapist (PT) to join our team. As a per diem position, you will provide direct patient care services, evaluate new patients, document treatment progress, coordinate care plans, and provide education and support as needed. This role offers the opportunity to work in a dynamic environment with a focus on patient-centered care and professional growth.

Responsibilities

• Provide direct patient care services, evaluations, and treatments.
• Document patient care data accurately and timely.
• Coordinate care plans and referrals with the interdisciplinary team.
• Offer in-service and public education, supervision, and orientation to support peers and students.

Requirements

• Bachelor's degree in Physical Therapy and current state licensure.
• Minimum one year of therapeutic experience preferred.
• Strong knowledge of physical therapy theory and practices.
• Continuing education and willingness to work in various conditions.
