Responsibilities

• Provide home-based care to patients post hospital, rehab, or nursing facility discharge.
• Collaborate with a team of healthcare professionals to oversee patient care and ensure positive outcomes.
• Conduct patient assessments, document progress, and communicate changes to physicians.
• Educate patients and families on care plans, medications, and home safety.
• Facilitate goal achievement and coordinate patient discharge.

Requirements

• Minimum 1 year of acute care experience; home healthcare experience preferred.
• Active RN license in FL and CPR certification.
• Familiarity with Oasis and state/federal home health regulations.
• Clinical proficiency, excellent communication, and compassion.
