Schedule: 4/10-hour shifts weekly, Day and Evening shifts, 8:30am-7:00pm
Salary: Competitive with potential $15,000 Sign-On Bonus
Job Summary
The Inpatient Case Manager, in collaboration with the clinical team and medical providers, is responsible for discharge planning coordination and intervention. This role focuses on promoting patient wellness, improving care outcomes, ensuring efficient health service utilization, and minimizing payment denials for patients with complex health needs.
Requirements
- Basic professional knowledge with a Bachelor's degree
- BSN required from CCNE accredited schools, waived for internal LBH candidates in specified roles
