Fall Community Health Plan is seeking a dedicated Physician to join our team at Summit ElderCare in Webster. Our Physicians play a crucial role in delivering personalized, quality care to older adults participating in the PACE program.

Responsibilities

• Provide clinical and social care to participants and caregivers.
• Work collaboratively with a team of geriatric care professionals.
• Contribute to individualized care plans through the Interdisciplinary Team approach.
• Promote dignity and autonomy of older adults.

Qualifications

• Medical degree with a focus on geriatrics.
• Valid medical license in Massachusetts.
• Board certification in Internal Medicine, Geriatrics, or Family Practice preferred.
• Previous experience with frail or elderly populations.
• Strong leadership and communication skills.
