Job Description
We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health. This position, based in Baltimore, MD at Levindale Hospital, involves providing clinical social work services, conducting psychosocial assessments, and offering counseling to patients and their families on various aspects such as family relationships, financial concerns, discharge planning, and support related to aging and end-of-life issues.
Responsibilities
- Conduct psychosocial assessments of patients and families.
- Provide counseling and support related to various adjustment issues.
- Collaborate with healthcare teams to facilitate discharge planning.
- Address emotional needs and offer psychological support.
Requirements
- Bachelor's degree and 1-3 years of experience in social work.
- LBSW/LMSW/LGPC certification.
- Strong interpersonal and communication skills.
- Ability to work compassionately with patients and families.
