SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. The position, based in Baltimore, MD at Levindale Hospital, involves providing clinical social work services and psychosocial assessments to patients/residents and families. Some of the common issues addressed include adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.

Responsibilities

• Conduct psychosocial assessments.
• Provide counseling on various social and emotional issues.
• Assist with discharge planning.
• Collaborate with healthcare professionals to ensure comprehensive care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed as an LBSW, LMSW, or LGPC.
• Ability to work empathetically and professionally with patients and families.
