Job Description

We are looking for a compassionate Care Coordinator (Wraparound) to join the Wisconsin Community Services team. In this role, you will provide intensive care coordination for children and adolescents under the Wraparound Practice Model of care. Your responsibilities will include maintaining a caseload of children and families, facilitating meetings, advocating for children and families in various settings, and collaborating with community resources.

Responsibilities

  • Maintain a caseload of approximately 8 children and families.
  • Contact enrolled children and families promptly and arrange initial visits.
  • Facilitate weekly visits, Child and Family Team Meetings, and maintain regular contact.
  • Assemble and coordinate the Child and Family Team, develop individualized Plans of Care.
  • Advocate for children and families across different settings.
  • Seek and provide community resources, transportation, and crisis/emergency services.

Requirements

  • Bachelor’s degree in a relevant field with one year of mental health services experience.
  • Completion of 85 hours of mandatory training in Wraparound philosophy.
  • Cultural competence, computer skills, and a valid driver’s license.
  • Ability to work with children, young adults, and families.
