Care Manager RN - Pediatrics

As a Care Manager RN, you will assess, plan, and coordinate medical and support services to ensure quality and cost-effective care for select members.

Responsibilities

• Assess member's health status, treatment plans, and services to develop a comprehensive care plan.
• Coordinate with healthcare providers and community services to meet members' needs.
• Provide patient and provider education, facilitate access to services, and monitor care management plans.
• Participate in care management rounds, review assessments, and ensure care plan compliance.
• Collaborate with team members on case management committees and special projects.

Requirements

• Graduate from an Accredited School of Nursing; Bachelor's degree in Nursing preferred.
• 2+ years of clinical nursing experience and 1+ years of case management experience in a managed care setting.
• Current RN license in Texas with knowledge of healthcare managed care and utilization management.
• Experience with medical decision support tools and government sponsored managed care programs.

Salary Range

$66,100.00 - $118,900.00 per year. Actual pay adjusted based on skills, experience, and job-related factors.
