Job Description

As part of the Homecare Division team, the Case Aide at RiseBoro Homecare will provide case assistance and coordination services to all participants of the Licensed Homecare Service Agency (LHCSA). In addition to developing a plan and communicating with staff members, clients, family, and/or caregivers to ensure necessary services are established for all clients, specific duties and responsibilities include:

  • Assisting clients in maintaining eligibility for homecare and Assisted Living Program (ALP) services
  • Maintaining complete, detailed, and accurate necessary client data in the designated client database/system
  • Providing accurate and appropriate information on services, benefits, and entitlements to clients
  • Providing comprehensive ongoing case management and coordination services, including but not limited to intake assessment, benefit assessment, goal setting, general information, case assistance, entitlement counseling, referrals, supportive contact, advocacy, and crisis intervention
  • Participating in the education of healthcare workers by attending in-service classes and facilitating discussions on Mental Health
  • Advocating and intervening with other agencies on behalf of a client
  • Completing client satisfaction surveys and participating in outreach efforts and events as needed
  • Performing other related duties as assigned

Competencies

  • Demonstrated knowledge of Homecare policies, procedures, and all governing regulatory bodies
  • Knowledge of community resources and counseling/social work practices with high-risk populations
  • Excellent interpersonal and customer service skills
  • Ability to maintain confidentiality at all times
  • Bilingual English/Spanish; ability to speak, read, and write in Spanish preferred
  • Proficient in Microsoft Office Suite, particularly Word and Excel

  • Ability to understand the concepts of institutional and structural racism and bias and their impact on underserved and underrepresented communities

Education and Experience

High School diploma or equivalent required with a minimum of four (4) years of professional work experience in healthcare, homecare, or human services preferred. Associate degree with a minimum of two (2) years of professional work experience in healthcare, homecare, or human services preferred.
