Job Description

The Case Aide will provide case assistance and coordination services to all participants of the Licensed Homecare Service Agency (LHCSA). Responsibilities include maintaining eligibility for homecare and Assisted Living Program (ALP) services, maintaining client data, providing comprehensive case management, participating in healthcare worker education, completing client satisfaction surveys, participating in outreach efforts, and fulfilling other related duties as assigned.

  • Ability to provide quality services with compassion, patience, and empathy
  • Bilingual English/Spanish; ability to speak, read, and write in Spanish preferred
  • Excellent verbal and written communication skills
  • Strong organizational and time management skills
  • Ability to work independently and collaboratively as a team
  • Proficient in Microsoft Office Suite, particularly Word, and Excel
  • Commitment to supporting underserved and underrepresented communities

Education and Experience

High school diploma or equivalent with a minimum of four (4) years of professional work experience in healthcare, homecare, or human services preferred. Associate degree with a minimum of two (2) years of professional work experience in the same field preferred. Any equivalent combination of education and experience that provides the required knowledge and skills may be considered qualifying.
