Job Description

We are looking for a dedicated Case Management Supervisor to join the St. Vincent de Paul Society of San Francisco team. As a Case Management Supervisor, you will play a crucial role in developing service plans to assist clients in securing and maintaining housing, employment, and essential resources for self-sufficiency. This position involves supervising staff, evaluating client needs, and ensuring the successful daily operations of the case management team at the Division Circle Navigation Center.

Responsibilities

  • Oversee SF Adult Coordinated Entry Case Management Services at Division Circle Navigation Center, ensuring clients achieve personal and programmatic goals.
  • Coordinate staff scheduling, case management assignments, and maintain 24-hour coverage of services.
  • Hire, supervise, and coach case management staff, including trainees, to provide comprehensive support services to clients.
  • Collaborate with internal and external stakeholders to meet program goals and deliverables.
  • Ensure accurate data entry and reporting, analyze program data for improvements, and maintain program-related documentation.
  • Conduct client assessments, develop and monitor case management plans, and provide direct client services as needed.

Requirements

  • BA degree in social services or related field required; master's degree preferred.
  • Four years' experience in clinical case management, with supervisory experience.
  • Excellent communication skills, ability to motivate staff, and lead by example.
  • Familiarity with Harm Reduction, Motivational Interviewing, and homeless issues.
  • Proficiency in MS Office, data entry, and reporting.
