HOSPICE MEDICAL SOCIAL WORKER

Job Description

We are currently seeking a compassionate and experienced Hospice Medical Social Worker to join the Liberty Healthcare Management team. As a Full-Time position covering Cumberland County, you will be responsible for providing direct patient care services, conducting clinical assessments, and developing personalized care plans to ensure the highest quality of care for our hospice patients.

Responsibilities

• Provide direct patient care services and document patient care data.
• Conduct clinical assessments and develop individualized care plans.
• Apply psycho-social principles and assess social, emotional, financial, and environmental factors affecting patient treatment.
• Collaborate with agency personnel and coordinate services for optimal patient care.
• Assist with homecare visits as required.
• Support patients and families through bereavement care and grief support.

Requirements

• Master's degree in Social Work from a Council on Social Work Education accredited school.
• Minimum of one year practical experience, preferably in home health/hospice care.
• Strong interpersonal skills and effective communication abilities.
• Willingness to travel and work under assigned conditions.
• Completion of a minimum of twelve hours of continuing education annually.
