Responsibilities

  • Provide inpatient palliative care, working directly with patients and families on end-of-life matters and care planning for the chronically ill.
  • Independently perform history and physical data, interpret and integrate data, and determine appropriate diagnosis and treatment plans.
  • Conduct quality of life discussions with patients and their families, order and interpret diagnostic and therapeutic tests, and collaborate with physicians in managing acute and long-term medical needs.
  • Prescribe pharmacologic and non-pharmacologic treatment modalities, maintain and review patient records, and educate patients and families regarding medications and treatment instructions post-discharge and for the home environment.
  • Exercise autonomy in medical decision making and provide a broad range of diagnostic and therapeutic services.

Qualifications

  • Hold current state RN and NP licensure, current Board Certification in a Nursing Specialty/Field, and a minimum of a Bachelors and Masters Degree in Nursing from accredited Colleges or Universities.
  • Experience in nursing is required, and some experience as a Nurse Practitioner/Provider is preferred, with demonstrated critical thinking skills.
  • Possess knowledge of Nursing and clinical Nurse Practitioner critical thinking skills application to the aged, chronically ill, and convalescent resident.
  • Strong leadership, interpersonal, and problem-solving skills, excellent written and oral communication skills, and traits of punctuality, organizational skills, and ability to function within a team.
