Job Description

We are looking for a dedicated and experienced Registered Nurse to join our team as an Intake & Hospital Liaison. In this critical role, you will be part of our Intake and Referral department, serving as the primary point of contact for patients and providers, ensuring a seamless transition to home health and hospice services. This position involves a blend of responsibilities including direct patient interaction, coordination of discharges, and clinical documentation review.

Responsibilities

• Educate patients and providers on available services.
• Coordinate patient discharges and transitions to home care.
• Review clinical documentation and assess patient eligibility for services.
• Write orders for care and provide information to patients, families, and community members.

Requirements

• Associate's degree in Nursing and RN license.
• Minimum of two years of experience as an RN.
