Registered Nurse (RN) - Home Health

Job Description

American Home Health Care is looking for a compassionate Registered Nurse (RN) to join our team on a full-time or part-time basis. The RN will be responsible for coordinating and directing patient care in Contra Costa County, ensuring individual patient needs are met with advanced assessment and decision-making skills.

Responsibilities

• Provide comprehensive assessment, planning, and evaluation for home care clients.
• Maintain accurate clinical records and documentation in accordance with agency policies.
• Deliver patient, family, and caregiver education.
• Coordinate patient care and identify discharge planning needs.
• Participate in on-call duties and oversee home health aide care.

Requirements

• Current RN License in the State of California.
• Minimum 1 year of recent experience in public health, home care, or hospice nursing.
• Current CPR certification and valid driver's license with insurance.

Compensation
