SOCIAL WORKER PRN

Job Description

We are currently looking for a motivated Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker PRN, you will provide clinical social work services, conduct psychosocial assessments, and offer counseling to patients, residents, and their families on various issues such as adjustment to institutional living, financial concerns, and end-of-life planning.

Responsibilities

• Provide clinical social work services to patients and families.
• Conduct psychosocial assessments and offer counseling on various common problems.
• Assist with discharge planning and end-of-life counseling.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
