Summary
JOB SUMMARY: Provides clinical social work services or psychosocial assessments to patients/residents and families, addressing various common issues including adjustment to institutional living, financial concerns, family relationships, medical understanding, aging, discharge planning, and end-of-life counseling.
Responsibilities:
- Conduct psychosocial assessments and develop treatment plans.
- Provide counseling and support to patients and families.
- Assist in discharge planning and transitions.
- Collaborate with healthcare professionals to address patient needs.
Requirements:
- Bachelor's degree in Social Work or related field.
- 1-3 years of social work experience.
- LBSW/LMSW/LGPC certification.
- Strong communication and interpersonal skills.
