Job Description
We are looking for a PRN Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, assisting them with various challenges and adjustments related to their medical care and emotional well-being.
Responsibilities
- Offer psychosocial support to patients and families facing issues such as institutional living adjustments and financial concerns.
- Provide counseling on topics including medical procedures, aging, discharge planning, and end-of-life care.
- Collaborate with healthcare teams to ensure holistic patient care.
Requirements
- Bachelor's degree in Social Work or related field.
- 1-3 years of relevant experience.
- LBSW/LMSW/LGPC certification.
- Strong interpersonal and communication skills.
