Job Description
We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. The role involves providing clinical social work services and psychosocial assessments to patients and their families, addressing various challenges such as adjustment to institutional living, financial concerns, and counseling related to death and dying.
Responsibilities
- Provide clinical social work services to patients and families.
- Conduct psychosocial assessments and address common issues.
- Assist with discharge planning and counseling on end-of-life matters.
Requirements
- Bachelor's degree in social work.
- 1-3 years of relevant experience.
- LBSW, LMSW, or LGPC certification.
