Job Description
We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As part of our Allied Health department at Levindale Hospital, you will provide clinical social work services and psychosocial assessments to patients and their families, assisting with various challenges and emotional support needs.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients and families.
- Address common issues such as adjustment to institutional living, financial concerns, and end-of-life planning.
- Collaborate with medical teams to develop discharge plans that meet the patient's needs.
Requirements
- Bachelor's degree in Social Work or related field.
- 1-3 years of relevant experience.
- Licensed as LBSW, LMSW, or LGPC.
