Job Description
We are looking for a passionate PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services to patients and their families, addressing a variety of common challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.
Responsibilities
- Conduct psychosocial assessments for patients and families.
- Offer counseling and support for issues related to medical diagnoses, aging, and discharge planning.
- Collaborate with interdisciplinary teams to ensure comprehensive care.
Requirements
- Bachelor's degree in Social Work or related field.
- 1-3 years of relevant experience.
- LBSW/LMSW/LGPC certification.
