Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide essential clinical social work services and psychosocial assessments to patients, addressing a wide range of emotional and social challenges.

Job Summary

In this role, you will assist patients and their families with various issues such as adjustment to institutional living, financial concerns, family relationships, and end-of-life care. Your responsibilities will also include discharge planning, counseling related to death and dying, and offering support during difficult times.

Requirements

• Minimum of a Bachelor's degree in Social Work
• 1-3 years of relevant experience
• LBSW, LMSW, or LGPC certification
