INPATIENT CASE MANAGER (RN)

Job Description

We are looking for a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. In this role, you will collaborate with the clinical team and medical providers to coordinate discharge planning and interventions for patients with complex health needs. Your primary focus will be on promoting patient wellness, enhancing care outcomes, ensuring efficient health service utilization, and reducing payment denials.

Responsibilities

• Coordinate and implement discharge planning in collaboration with the clinical team and medical providers.
• Promote patient wellness and optimize care outcomes.
• Ensure efficient utilization of health services.
• Minimize denials of payment among patients with complex health needs.

Requirements

• Basic professional knowledge with equivalent to a Bachelor's degree.
• BSN required from CCNE accredited schools.
• Prior experience in care coordination or case management is a plus.
